Property:

•  A photocopy of the Binder or Contract of Sale Agreement,    signed by all parties (including all addendums).
•  A photocopy of the warranty deed if refinancing.

Employment and Income :

•  Photocopies of paystubs for the most recent 30 day period,
   and most recent two years W-2 forms.
•  Applicants who are paid by commission should provide    their two most recent tax returns signed and dated in ink.    (Form    1040).
•  Self employed applicants should provide their two most recent    years tax returns including all schedules. Provide Partnership    and
•  Corporate tax returns, if applicable. In addition a year to date    profit and loss statement and balance sheet (less than 90 days    old).

Assets:

•  Photocopies of three most recent bank statements (all pages)    showing account name, address, account numbers and    balance of checking and savings accounts.
•  For stocks, bonds and investment accounts: a photocopy of    Broker's Statement (covering a three month period - or most    recent three months statements).
• Gift Letter and verification of donor's funds from relative. There    must be proof of donor's ability to give the gift (i.e. bank    statement or canceled check).
